*Any request for home quarantine made less than 72 hours before the departure of the flight to French Polynesia will not allow its instruction.
This form allows the public authorities to control the conditions under which you intend to carry out your quarantine upon arrival in French Polynesia.
In accordance with article 441-7 of the Penal Code, filling out this administrative form with false information is punishable by one year of imprisonment and a fine of 15,000 euros (equivalent to 1,794,000 F CFP).
Controls will be carried out by police forces.
PROTECTION OF PERSONAL DATA
Your personal data, collected on the ETIS platform, are subject to a processing whose purpose is the implementation of the formalities required for the entry and health monitoring of arrivals in French Polynesia in the context of the fight against the COVID-19, as set by Decree n°525 CM of May 13, 2020 as amended.
This treatment is implemented by the Tourism Department and the Health Department, administrative services of French Polynesia. It is based on the country's public interest missions relating to the management of health threats and the protection of health.
Treated data :
The data collected are your identity, date of birth, nationality, passport number and, if applicable, those of your minor children (traveling with you or alone), your contact data (geographical addresses, e-mail address, telephone number ...), your travel itinerary, your commitment to comply with applicable testing and health security measures and your login data to the platform.
Are also processed, the data related to the fulfilment of the quarantine (place, conditions and commitments to carry out the quarantine at home or in a dedicated center).
The data to be informed on the platform are mandatory for any person entering the territory of French Polynesia. If you do not fill in the online form, your request will not be processed.

The data is at destination :
- from the tourism department for the entry formalities in French Polynesia;
- from the Directorate and the Ministry of Health for monitoring quarantine measures and traveler identification needs in the event of infection by the COVID-19 virus.
The High Commission of the Republic in French Polynesia receives the data enabling it to evaluate requests to carry out home quaratine and, if necessary, to oppose it in accordance with Order No. 525 CM of May 13, 2020 as amended.
The necessary data for dedicated center are communicated to the general sources department, administrative service of French Polynesia, in charge of the organisation of the accomodation.
The ETIS platform also allows the creation of QR codes (bar codes) transmitted to the laboratory of the Louis Malardé Intitute to allow the distribution of screening tests to travelers. The laboratory does not access your data on the platform.
Duration of data retention :
Your data is kept for a maximum of 90 days and then destroyed.
Anonymous data is kept by the establishment of statistics on tourist frequentation.
The platform is developed and maintained by the company prox-i. It is hosted by the company OVH.
