Il governo della Polinesia Francese richiede la creazione di un modulo di registrazione sanitaria per ogni viaggiatore in arrivo per via aerea nella Polinesia Francese. Questa formalità si aggiunge già esistenti per gli stranieri in arrivo nel territorio polinesiano.. Questa formalità è in essere dal 12 luglio 2020 (Decree No. 1335 CM of July 21, 2021 amending Decree No. 525 CM of May 13, 2020, as amended, on entry and health surveillance measures for arrivals in French Polynesia in the context of the fight against covid-19.) e rimane valida fino a revoca.

Il formulario sanitario è disponibile esclusivamente sul sito www.etis.pf. ed è obbligatorio per tutti i viaggiatori residenti e non residenti e deve essere presentato al banco check-in della compagnia aerea prima dell'imbarco sui voli per la Polinesia.

Chi deve farne richiesta?

All travelers; residents and non-residents; as well as their minor family members are subject to this declaration obligation.
Please fill out the appropriate form according to your Country of Departure and vaccination status.

Quali sono i passi
da seguire?
  • Take a molecular RT-PCR test of SARS-CoV-2 performed on a sample by an authorized health professional within three days before boarding for all travelers aged 11 years old and older. Failling to obtain the result in due time, make sure to present to the transport company a certificate of performance of the test associated witha negative antigenic test result taken no later than 48 hours prior to the departure. Please note that the only antigenic tests which can validly be presented are those allowing the detection of the N protein of the SARS-CoV-2.
  • Present the results of the test to the airline serving French Polynesia
  • Fill out entirely the online health commitment form on the www.etis.pf website
  • Accept the operative health conditions.
  • Depending on your vaccination status, present your test results and vaccination record.​
  • Pay a contribution to the sanitary surveillance charges for the amount of 5 000 CFP per traveler (from 6 years old).
Che cosa serve per questa richiesta?
  • The negative RT-PCR test result taken no later than 72 hours before departure or the negative antigen test taken no later than 48 hours before departure. Please note that the only antigenic tests which can validly be presented are those allowing the detection of the N protein of the SARS-CoV-2.
  • A travel itinerary.
  • A valid email adress on which the ETIS receipt will be sent
  • A person to contact in case of emergency, phone number and email adress.
  • Travel insurance for anyone who does not have the French nationality. If the insurance doesn't cover the costs in the event of a pandemic, engage yourself to take care of all health costs generated in French Polynesia, including in the event of infection with COVID-19. The etis form holds as proof. ​
  • Your validated compelling motive request for the travel submitted between 30 and 6 days prior to the flight on the Démarches Simplifiées ( or simplified steps) plateform. Please note that if you do not have the validation fo your compelling motive, you will not be allowed to board and you will need to make a request on simplified Procedures platform.
  • Test results depending on your immunization or immunity status. ​
